Can anyone help me find a form for family preparedness?

Maybe like, a form that you can list your emergency numbers and places to hide in the house. Something with good detailed info, but can be compacted down into 3 or less peices of paper. Im getting into the whole 'family preparedness' thing.

1. Below are three links for you to check out. You didn't specify any particular disaster so there are some slight variations. One thing to add that we have is a family Yahoo account that anyone in the family has access to besides their own personal account. This allows all a central contact. Besides writing down credit card numbers you will want to write down serial numbers of valuable items. You will also want to video all your possessions store the video on an online site so if everything went you have documented proof of what you owned. I hope this helps. Good luck
